State Reporting

Districts are required to submit various reports in order to receive funding from the state. Explanations of the most common reports are listed below.

Enrollment

Reporting of school district enrollment eligible for state funding. Forms can be submitted electronically and/or on paper.

  • P223 Enrollment Eligible for Basic Support
    Headcount and Full Time Equivalent (FTE) count of Kindergarten through 12th grade basic education enrollment. This form is also used to report bilingual education and the running start program enrollment.
  • P223H Special Education Enrollment
    Special education students reported need to met the following requirements on the count date: (1) the student is an enrolled student of the school district. (2) the student has a current individualized education program (IEP). (3) The student has been reevaluated every three years. (4) The student is currently receiving special education services.
  • P223S Nonstandard School Year Enrollment Eligible for Basic Support Nonstandard school year average annual full time equivalent (AAFTE) enrollment eligible for basic support. School districts providing instruction during the months of June, July, and August may claim basic education support if the following conditions are met: (1) the student is enrolled in tuition free courses of study that meet basic education requirements. (2) Nonstandard school year enrollment is in lieu of enrollment during the regular school year inclusive of enrollment in a private school or home-based instruction. (3) The student's nonstandard school year enrollment when combined with the enrollment in the first nine months of the school year including enrollment in private school or home base instruction does not exceed the limitations on enrollment counts. (4) The enrollment is not claimed for separate summer skills center funding. (5) The enrollment in a community or technical college is not in a running start program.
  • P240 Ancillary Services to Part-time Students Eligible for Basic Support Monthly attendance report of ancillary services to part-time students eligible for basic support. Ancillary services are any co-curricular service or activity, any health care service or activity, and any other services or activities, for or in which enrolled students are served by appropriate school district staff. Districts report the actual number of student contact hours of ancillary service for part-time, private school, and home-based students.

Enrollment Report Count Dates and Due Dates
  • Monthly count dates - fourth school day of September and the first school day of October through May.
  • Monthly due dates - Tenth school day of September and the tenth calendar day for October through August.
  • P213 Students Residing in Nonhigh District and Enrolled in High School District
    Report of students residing in nonhigh districts and enrolled in high school districts. This is a yearend report and the form must be submitted on paper. Any high school district enrolling students residing in a district that does not offer the grade level they are enrolled in should be reported on the form. High school districts complete separate P213 forms for each nonhigh district served and send the signed form(s) to the appropriate ESD by deadline. ESDs forward each P213 form to the appropriate nonhigh district for a concurring signature. Nonhigh districts review and sign the form and return it to the ESD by deadline.

    High District June 10, 2008
  • P210 Public High School Enrollment Status & Vocational End-of Year Enrollment
    Report of every student who was enrolled in grades 9 to 12 in the public school system at any time during the reporting period. The reporting period is defined as the first day of school in the fall of the school year to the day before the first day of school in the fall of the next school year.
    Due Date: December 3, 2007
  • E525 Home and Hospital AttendanceReport of final home and hospital attendance, and must be submitted on paper. Days of services are converted to weeks and reported in two categories. Enrollment at sites with a single students, such as in the student's home or hospital room, and enrollment sites with multiple-enrolled students such as in a children's ward of a hospital or a residential treatment center.
    Due Date: July 10, 2008
  • P105A School Enrollment Report (Ethnic)
    Report of unduplicated enrollment by grade level, gender, and racial/ethnic origin for each school in the state. Headcount of every student a district serves.
    Count Date: October 2 and April 2

  • Personnel Reporting (S275)
    Electronic personnel reporting of each employee who, as of October 1, is contracted or hired to provide services any time during the school year.
    Due Dates:
    October 1, 2007 Snapshot date for all staff
    November 20, 2007 Initial Submission
    December 31, 2007 Corrections to edit messages for Snapshot date
    August 31, 2008 Final district updates to current items
    September 30, 2008 Final Corrections to edit exceptions

  • Revenue Estimates (F203)
    Based on district information, the potential state funding is computed. These computed revenues are compared to the amounts actually used in the budget. During the school year the F203X option is available to estimate revenues based on proposed changes in state funding for the upcoming school year.
  • Budget (F195)
    Estimate of revenues and expenditures for a district for the school year. The expenditures are the legal limit on spending.

    July 10 Budget available for public and ESD review
    August 1 Final date for adoption of budget
    August 3 Final date for to forward budget to ESD
    1st Class Districts
    July 10 Budget available for public and ESD review
    August 31 Final date for adoption of budget
    September 3 Final date to forward budget to ESD

  • Budget Extension (F200)
    Used when the legal limit on spending is not adequate.
    Due Dates:
    2nd Class Districts
    August 1 Final date to give public notice of extension
    August 5 Extension available for public review
    August 15 Final date to adopt extension
    August 20 Final date to forward extension to ESD
    1st Class Districts
    August 1 Final date to give public notice of extension
    August 15 Extension available for public review
    August 31 Final date to adopt extension
    August 31 Final date to forward extension to ESD

  • Year End Reporting (F196)
    Report of the financial activity of the district for the school year. This report is required by the state and is used to determine if state funds should be recovered.
    Due Dates:
    October 25 Final date to submit F196 to ESD for review
    November 1 Certification page submitted to school district
    November 8 Signed certification page due to ESD
    November 15 Final date to submit signed certification page to OSPI
